The followings are the terms & conditions for Zazen Health Solutions Privilege Card.
- This Card acts as a privilege and loyalty card given to its customers by Zazen Health Solutions Sdn Bhd [731466-X)] and is only valid at all ZHS Participating Outlets in Malaysia. A list of all ZHS Participating outlets can be found at www.zazenhealthsolutions.com.
- This card is not applicable with other discounts or promotions and any programme arranged by third parties such as credit card companies.
- This Card is non-transferable and only entitles the cardholder whose name and identification appears on the Application Form to the benefits and privileges accorded by the Company, subject to the terms and conditions as described therein.
- This Card is valid for a lifetime and is strictly a means of identifying the cardholder’s entitlement to the privileges prescribed and is not to be used as a charge, credit or authority card.
- The Cardholder is responsible for advising and updating the Company of any changes to particulars pertaining to his or her contact details.
- This card remains the property of the Company and must be surrendered to the Company upon request. If any lost Card is found, it should be returned to the nearest ZHS Outlets.

You need JavaScript enabled to view it . A fee of RM30 will be levied to replace lost or damaged Privilege Card. The replacement period for defects on the card is only valid for one (1) month after the card has been issued to the cardholders.
